Under the general direction of the Pharmacy Manager of Clinical Services, or their designee, the Clinical Pharmacist reviews drug therapy; formulates recommendations ba! sed on current literature and established standards of practice; serves as a resource for drug information; and participates and directly manages the preparation and distribution of medications. Additional responsibilities include the direct supervision of support personnel along with serving as a member of the healthcare team with the prescriber and direct caregiver.
You will provide safe and comprehensive care for patients on the Adult Inpatient Medicine and Psychiatric Units based on a team approach. Core functions include: rounding with Medicine/Psychiatric teams, pharmacokinetics consults, participation in multidisciplinary rounds, intervention reporting to include IV to PO changes, patient discharge counseling, drug dosing adjustment for renal impairment, and acting as preceptor to pharmacy students.
Requirements:
- Graduate of an ACPE accredited school/college of pharmacy
- Completion of ASHP-accredited PGY-1 Residency or equival! ent years of hospital experience
- Excellent verbal and! written communication skills
- Eligibility to obtain or currently licensed in the State of Maine
- Analytical skills to identify and resolve problems related to drug therapy
- Drug therapy monitoring and consulting for patients of all ages
- Work experience as a patient care pharmacist involved in patient monitoring and drug therapy review is preferred.
