Clinical Pharmacy Specialist - Inpatient Anticoagulation
Position Summary:
A clinical pharmacist specialist position working to prov! ide safe and quality comprehensive anticoagulation care for inpatients at Maine Medical Center . Some core functions are as follows: Coordinate the development and implementation of anticoagulation management activities to ensure the consistent delivery of safe and high quality patient care. Establish an educational practice to enhance organizational clinician competency on all topics related to the use of anticoagulants. Oversee quality assurance programs to identify areas of potential improvement and level of risk. Collaborate with nursing and other disciplines to ensure that optimal patient discharge counseling occurs. Conduct medication formulary activities as needed on topics related to anticoagulation.
Candidate qualification requirements or preferred experience are: Eligible for pharmacist licensure in Maine . Doctor of Pharmacy degree required. PGY1 pharmacy practice residency required. PGY2 pharmacy residency or equivalent experience (3 years) in either ca! rdiology or anticoagulation management is highly preferred. Pr! eference that candidate is a Certified Anticoagulation Care Provider (CACP).
Summary of Position Description
Under the general direction of the Pharmacy Manager of Clinical Services, the Clinical Pharmacy Specialist reviews drug therapy; formulates recommendations based on current literature and prevailing opinion; participates in the education of physicians, pharmacists,nurses, and allied health professionals; serves as a resource for drug information; pariticpates in drug research trials based on specialty and current relationships with principle investigators; and serves as a liasion between the specialty and department operations. Active participant on the Pharmacy & Therapeutics Committee.
Minimum Knowledge, Skills, and Abilities Required
Work experience as a patient care pharmacist involved in patient monitoring and drug therapy review is required.
Excellent verbal and written communication skills are required.
Gra! duate of an ACCP accredited school/college of pharmacy is required.
Eligible or currently licensed in the State of Maine.
Experience in an academic setting as an educator of pharmacists, physicians, nurses, and allied health professionals is required.
Experience in the monitoring and consultation on drug therapy in patients of all age groups.
Experience in the design, enrollment, monitoring, documentation, and analysis of patients in research drug protocols is preferred.
Excellent presentation skills on topics of clinical relevance.
