Care Team Medical Assistant
Reports To: Practice Director and/or Clinical Coordinator
Supervises: Not Applicable
Status: Hourly, Non-Exempt
POSITION SUMMARY
This position is responsible for assisting with the smooth and efficient day-to-day operations of the clinical team in the delivery of primary health care and population health management.
ESSENTIAL JOB FUNCTIONS
Under! stands and provides direct patient care, meeting the standards! and requirements of the medical assistant job description.
Reviews various reports to identify gaps in care for the providerâs patient panel and assists patients in getting the appropriate care.
Identifies patients who may benefit from assistance from other members of the care team (pharmacists, care managers, therapists, social workers, physical therapist, nutritionist etc.) and links them with this support.
Collaborates with providers to facilitate the ordering, completion of, and result flow-sheeting including, but not limited to, labs, immunizations, consults and other diagnostic testing.
Provides pre-visit planning for upcoming patient visits for all members of the providerâs panels.
Huddles daily with other members of the care team to plan their approach to the day and prepare for challenges, such as complex patients who require additional time and resources.
Assist with providing patient and family with optim! al linkage to PCHC services and community resources by facilitating referrals to the PCHC referral department.
May assist with billing duties, including entering charges and diagnoses, and following up with providers on incomplete information.
Utilizes the Electronic Medical Record efficiently to document all patient interactions.
Discusses and clarifies issues with patients at the end of their appointment and also provides patient education as necessary.
Interacts harmoniously and effectively with others, focusing upon the attainment of organizational goals and objectives through a commitment to teamwork.
Conforms to acceptable attendance and punctuality standards as expressed in the Employee Handbook.
Complies with all safety rules and protocols, as established by the Environment of Care Committee. Immediately reports any workplace injury to supervisor.
Abides by the organizations compliance program and requir! ements.
Current on all required training for current year.
Performs all other duties, as assigned by supervisor.
