DUTIES:
The Clinical Pharmacy Specialist must be capable of using professional judgment and clinical expertise to solve complex problems and to provide clinical information to Healthcare providers and patients.

Clinical:
·

Use advanced knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics, and serve as an authority on pharmaceutical care in specialized and routine areas of clinical pharmacy practice for medical staff, nursing staff, and pharmacy staff.

·

Evaluate, monitor and manage medic! ation of patients referred by consult to the Clinical Pharmacy Speciali! st for chronic disease state management. Specifically, these disease state management activities includes:

The design of a therapeutic regimen or plan, based on patient-specific information, which includes establishing a therapeutic endpoint, selecting and adjusting

appropriate drug therapy, establishing an appropriate dosage regimen and providing appropriate and timely lab monitoring related to all prescribing activity.

·

Review of medication regimens of all referred patients and noting for the provider any findings that might require action that include but are not limited to:

Inappropriate dose, dose interval, or dosage form; Drug and/or pharmacologic duplication; Adverse drug-drug, drug-lab, or drug-food interaction; Adverse drug reactions (reports adverse drug events (ADE), near misses and, medication errors in alignment with computerized reporting program; Inappropriate therapeutic regimen; Misuse of medication, i.e., abus! e, overuse or underutilization.

Serve as a drug information resource for Ambulatory Care/PACT providers answering inquiries regarding therapeutics, drug interactions, dosing, side effects, formulary options, etc.

·

Assess or provide patient care considering the age of the patient as it relates to ability to understand directions, provide objective self assessment or any other medical milieu appropriate to the patient's overall treatment plan

·

Performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of medications.

This may include adding medications or discontinuing duplicate medications or those the patient is not taking

Administrative:
·

Timely review of electronically-submitted non-formulary drug requests (NDR) utilizing National PBM

Criteria for Use.

·

Participat! e and/or initiate quality management and research efforts within Pharma! cy Service, including analysis and management of a clinical practice within an area of expertise or as otherwise assigned. Assist in conducting and reporting internal audits as required including computerized reporting.

·

Participate and/or support Pharmacy Service-initiated cost/benefit programs in support of medical center's efforts at maintaining stewardship of resources.

·

Participate in peer review as stipulated by directives and policies for clinical pharmacists providing direct patient care under a scope of practice.

Education:
·

Assume an active role in the overall pharmacy-training program including pharmacy students, pharmacy residents; professional and non-professional staff members, clinical pharmacy meetings, and journal club.

·

When selected to precept Pharmacy Residents, participate fully in providing feedback, evaluation and documentation of Pharmacy Residents utilizing co! mputerized software.

·

Provides in-service educational programs (written or verbal) to selected Pharmacy Service and medical center staff and providers at the direction of the Clinical Coordinator and/or Chief of Pharmacy.

WORK SCHEDULE:
Monday - Friday, 8:00 am to 4:30 pm.

On call, rotating weekend and holiday coverage, 7:00-3:00pm (approximately once every 12 weeks and once every 12 holidays)

Preferred
Experience

: PharmD degree, post-graduate year one (PGY1)
residency training or equivalent experience, Board Certification, experience in
primary care (diabetes, hypertension, hyperlipidemia, anticoagulation
therapy/monitoring), pharmacokinetic monitoring, active participation in
residency training programs, clerkships, internships, and staff development
programs.

GRADE DETERMINATIONS

GS-13

At the

GS-13

level, in addition to
the basic requirements, candidates! must have an equivalent of 1 year of
creditable experience equivalent to the next lower grade level GS-12, which is
directly related to the position to be filled.

Experience:
To be creditable,
the experience must have demonstrated the use of knowledge, skills, and
abilities associated with professional pharmacy practice; communication orally
and in writing with patients and healthcare staff; knowledge of laws,
regulations and accreditation standards related to the distribution and control
of scheduled and non-scheduled drugs and pharmacy security; skill in monitoring
and assessing the outcome of drug therapies, including physical assessment and
interpretation of laboratory and other diagnostic parameters.

NOTE: Professional practice means
paid/non-paid employment as a professional or unlicensed graduate pharmacist as
defined by the appropriate licensing board. Qualifying experience must be at a!
level comparable to pharmacy experience at the next lower level. E! xperience as
a Graduate Pharmacist is creditable provided the candidate was used as a
professional pharmacist (under supervision) and subsequently passed the
appropriate licensure examination. Part-time experience as a professional
pharmacist is credited according to its relationship to the full-time
workweek.

In
addition to meeting the basic requirements, pharmacists at the

GS-13

level must demonstrate the following Knowledge, Skills and Abilities (KSAs):

Ability to
communicate orally and in writing to persuade and influence clinical and
management decisions.

Expert understanding of regulatory and quality standards for their
program area.

Ability to solve
problems, coordinate and organize responsibilities to maximize outcomes in
their program area of clinical expertise.

Advanced skill in
